It is no longer that simple. I interviewed Dr. Robert Vaschak, who explained why physician practices, now employ Nurse Practitioners (NP’s) and Physician Assistants (PA’s). Both are trained and licensed to provide care for many disease processes; and they can render care equivalent to that of a doctor, for a specific level of routine care. Dr. Vaschak listed several of the demands on health care professionals that has led to the “team approach” of using NP’s and PA’s as part of patient care. Due to space limitations, I will only cover two of the causes. 1) Easy to Understand Patient Notes. Doctors/PA’s/NP’s have to document in a manner that is understandable for the patient. This has increased the amount of time spent on documenting on a patient’s office visit. 2) Wellness Visits Required. Many insurance companies /Medicare programs penalize doctors for not performing wellness visits - at any age. In turn, it displaces time otherwise spent for follow-up visits with patients who have chronic health problems (i.e., high blood pressure, diabetes, congestive heart failure, etc..). Vaschak explained, “This is why there is a team approach for primary care.
