National Health Care Decisions Day is April 16,
2016. Serving Our Seniors & Firelands Regional
Medical Center are holding a public forum on April
12th (see page 5 Calendar of Events) to encourage
everyone to review, update, or create their own
durable power of attorney for healthcare.
What is a durable power of attorney for health
care?
It is a legal document that authorizes
another person to obtain your health information
and make heath care decisions for you, should you
be unable to make those decisions in the future.
It also requires the person you appoint to make
decisions that are consistent with your wishes.
Why should everyone have a durable power of
attorney for health care?
It allows your plan for
healthcare to continue according to your wishes
after you are not able to make them. It provides
peace of mind that your decisions will control
your care. Even further, it relieves the pressure
of your loved ones from having to make decisions
on your behalf.
Do I have to draft a durable power of attorney
for healthcare on my own?
No. The Ohio State
Bar Association has drafted easy to fill out forms
for your use. The forms are available at Firelands
Regional Medical Center and Serving Our Seniors
office.
What do I do after I fill out these documents?
Make several copies. Give a copy to a trusted
member of your family or friend. Keep a copy with
your personal papers. Leave a copy with your
physician and your lawyer, and even with your
clergy person. Most importantly, give a copy of
your health care power of attorney document to the
person you have designated to be your agent.
